Initial Contact: When you feel the need for help in handling daily tasks due to illness or old age, get in touch with us. We’ll schedule an appointment at a time that suits you.
Assessment Meeting: During the assessment, we’ll discuss your needs and how we can support you. We’ll also explore digital solutions that can help maintain independence.
Choosing Service Options: After the assessment, you can choose from three service options that vary based on the level of support needed.
Two-Week Familiarisation Period: Once you're happy to proceed, there's a two-week period for you to become accustomed to the services, with adjustments made as needed.
Service Review: After the two-week period, we’ll have a review to get your feedback on how things are going and what can be improved.
Finalising the Care Plan: Taking your feedback into account, the service will be formalised, and a care plan will be created detailing what you can expect, when, and how.
Digital Care Plan: A paper copy care plan will be placed at the service user's residence, and a digital copy will be made available to loved ones.
Carer Visits and Record Keeping: Carers will visit as per the care plan and maintain records of each visit, including the time, activities, and mood of the client.
Online Portal Access: The daily records will be accessible on a secure online portal for both the client and their loved ones.
